Our Commitment
Healthview Therapy Centre is committed to respecting patient privacy and protecting their personal information. This Privacy Statement explains the current information management practices of our organization to ensure that your personal health information is private, confidential and secure.
Privacy Officer
The position of privacy officer for Healthview Therapy Centre shall be held by the office administrator. The duties of the privacy officer include maintaining knowledge of current regulations, ensuring that the privacy policy is in keeping with these regulations, educating staff members on privacy policies, ensuring policies are followed, and arbitrating any complaints.
Collection and Use of Information
Healthview Therapy Centre collects personal and health information of patients in order to effectively treat the patient and, when required, to bill third party insurers. This information is stored in individual patient files on the office computer.
Information Handling and Storage Procedures
1. All files are to be stored in an area that is accessible only by staff members of Healthview Therapy Centre.
2. When in use, files are to be placed in such a manner as the patient name is not visible to the public.
3. Any paper to be disposed of that contains patient information must be shredded.
4. Staff members are not to reveal any patient information except in those situations outlined in the section Disclosure of Information.
5. Information stored electronically must be password protected or stored in a secure location.
6. All electronic information must be purged before disposing of a hard drive.
7. All information that is faxed or emailed will include a confidentiality statement.
8. Clinic building where files are stored is to be protected by a monitored alarm system.
Disclosure of Information
1. Patients may request a copy of their chart to be provided to them within 2 working days. There will be a charge of $45 plus $0.21 per page.
2. Patients may request in writing that their health information be sent to another health practitioner.
3. Information may be disclosed to third party insurers for the purpose of billing health services. These insurers include but are not limited to: private insurance companies, Manitoba Health, Worker's Compensation Board, and Manitoba Public Insurance.
4. Information may be released to government or law enforcement representatives when there is a legal obligation to do so.

Destruction of Information
All health information must be kept for a seven year period. After this time Healthview Therapy Centre may choose to store files in a secure location or destroy obsolete files. Should a file be destroyed, it will be shredded to protect private information. A record of the destruction will be kept on file.
Security Breaches and Corrective Procedures
All security breaches must be reported to the privacy officer. The privacy officer will determine if privacy policies were followed and if any policies need to be changed or amended. The privacy officer will reprimand anyone involved in a security breach and punitive measures may be employed. Patient generated complaints regarding privacy are to be adjudicated by the privacy officer in a timely manner. The complainant will be informed of what measures were taken to address their complaint and where appropriate be provided with a written apology.
